Sindhuja had come for review the same day we spoke to Dr. Balasundrum about the ROTI CHILD PROGRAM.

Her problem is much more complex than an ASD or VSD. I have attached an explanation, which you may wish to share on the ROTI child website. Pictures from India, should be here ASAP.

As soon as the Rotary Gov. from Srilanka returns home he has promised to contact our doctor to get us a child's chart from his district.

I doubt that the budgeted amount will be sufficient for Sindhuja because of excessive complications, but Dr. Balasundrum  has spoken to a few other people who have promised monetary support to make the difference.

Her case is very compelling! It must be horrible to be 12 yrs. old and to have experienced ill health, constant colds and infections all your life. She should have had the operation as a baby. Normally, in America, Surgical Correction is done for TOF between the ages of 6-12 MONTHS!!

EXPLANATION OF CHART:

On the left hand side is a normal heart.

Normally deoxygenated "used" blood flows from all over the body through the veins, back to the heart.

(BLUE- deoxygenated)

From the Right Atrium (RA) to Right Ventricle (RV) .

From the RV to the Lungs via the Pulmonary Arteries (PA).

In the lungs the blood is "recycled" and fresh oxygen added. (RED- oxygenated)

From the lungs this oxygenated blood flows into the Left Atrium (LA) through the Pulmonary veins.

From the LA to the Left ventricle.

Finally, the "good" oxygenated blood is pumped to the rest of the body from the LV via the Aorta (AO)- the largest artery.

Blood only flows in one direction and back flow is prevented by the presence of valves.

The Left picture shows what happens in a child with TOF.

There are FOUR main abnormalities.

1: A Ventral Septal Defect (VSD) i.e. an abnormal hole in the wall or "septa" that divides the right and left ventricles. So "bad" deoxygenated blood from the right ventricle (RV) mixes with the "good" oxygenated blood in the Left ventricle.

    2: There is a narrowing of the valves leading to the pulmonary artery (PA) from the Left Ventricle (LV) – pulmonary stenosis. So less blood flows to the lungs.

3: The Aorta "overrides" or straddles the hole in the septa. So "bad" deoxygenated blood from the Right Ventricle RV which has been mixed with the oxygenated blood from the left ventricle LV, flows into the Aorta and is pumped to the rest of the body – (PURPLE- mixed blood).

4: The final abnormality is thickening (hypertrophy) of the muscular wall of the right ventricle.

The symptoms are present from birth and include:

Difficult feeding (poor feeding habits)

Failure to gain weight

Poor development Cyanosis which becomes more pronounced during periods of agitation

Passing out

Sudden death

Clubbing of fingers (skin or bone enlargement around the finger nails)

Squatting during episodes of cyanosis

Repeated infections

Shortness of breath aggravated by exercise

The reason Sindujas problem is more complex is that she ALSO has an Atrial Septal Defect ASD, an abnormal hole in the wall or "septa" between the right and left atria.
